Facilitating Multi-Sector Collaboration

Often, society's best of intentions to serve those who are most vulnerable creates a reality where care and services are uncoordinated, complicated to understand, and difficult to improve. Sometimes, the hardest question to answer is "Where to begin?" We can help. We facilitated the design and launch of Wisconsin's first health home for children in foster care. Enrollment in this program ensures universal care coordination services are available to help biological families, foster families, and child advocates from all sectors achieve the best possible outcomes for these vulnerable children. ​At the close of the third year of operations, performance exceeded benchmarks in all targeted metrics, including immunization rates, lead screenings, rate of emergency department utilization, preventive dental care services, and mental health screenings.
